Body contouring surgery is not a substitute for weight loss.
These procedures are generally considered when localized fat, excess skin or changes in body contour remain despite relatively stable body weight.
Liposuction removes selected areas of subcutaneous fat to improve body contour.
It is not an obesity treatment and cannot remove visceral fat located inside the abdominal cavity.
Depending on the patient’s anatomy, liposuction may be planned for areas such as the abdomen, waist, flanks, back, arms or thighs.
Technologies such as PAL or ultrasound-assisted liposuction can be useful surgical tools, but technology alone does not determine the outcome.
Patient selection, anatomical planning and surgical technique remain fundamental.
A tummy tuck, or abdominoplasty, may be considered when pregnancy, significant weight changes or aging have resulted in excess abdominal skin and, in some patients, separation of the abdominal wall.
Surgery may involve removal of excess skin, repair of diastasis recti when appropriate and liposuction of selected areas when it can be performed safely.
A tummy tuck is not a weight-loss operation or treatment for obesity.

Overall health, BMI, smoking or nicotine use, expected operating time and individual risk of complications—including venous thromboembolism—should be considered when deciding whether procedures can safely be combined.

Gluteal contouring with fat transfer requires both aesthetic and safety planning.
Fat is harvested from selected donor areas through liposuction, processed and transferred to appropriate areas to improve body proportions.
In Dr. Atilla Fesli’s approach to BBL, fat transfer is planned within the subcutaneous plane rather than the gluteal muscle, with safety taking priority over maximum volume.
Not every patient has enough donor fat to achieve a significant increase in volume, and the amount of transferred fat that survives can vary between individuals.
Following significant weight loss, aging or changes in skin elasticity, some patients develop excess skin of the upper arms or thighs.
When skin elasticity is adequate and localized fat is the main concern, liposuction may sometimes be sufficient.
When significant loose skin is present, however, arm lift (brachioplasty) or thigh lift surgery may be required.
These procedures involve scars, and the location and length of those scars depend on the amount and distribution of excess skin.
Not every sign of facial aging requires surgery.
Depending on the patient’s anatomy and goals, non-surgical options may include treatments such as botulinum toxin, fillers, skin boosters, mesotherapy or selected energy-based treatments such as HIFU.
However, these treatments address different problems.
Botulinum toxin does not perform the same function as filler; filler does not reposition tissues like a facelift; and HIFU or mesotherapy should not be presented as surgical facelift alternatives for every patient.
The first step is identifying whether the main concern involves skin quality, muscle activity, volume loss or tissue laxity.
Only then can an appropriate treatment be selected.
